@cato There was an incident a couple years ago at a power plant in South Africa - basically a turbine overspeed test where the generator was electrically disconnected, the turbine went into an insane overspeed without stopping, and blew itself and some of the turbine hall apart.

A heavy steel part, I think part of the coupling between the generator and the turbine, got thrown through a brick wall into an electrical equipment room.

unless someone cut a hole with an oxyacetylene torch, and then decided to make it bigger by cutting out petals around the perimeter and bending them inward

either way looks intentional
